  • Information, support and referral for services by Public Health Nurse
  • Pediatric Neurology Clinic held in Montrose
  • Helps with resources and coordination for families with children with birth defects, medical condition, or family situations which put them at risk for delays.

  • For children or youth birth to 21 years of age who have, or are at risk for, conditions that impact their well-being.
  • Connects families to services.
  • Information and resource referral.
  • Nurse supports families to manage child's special needs including physical, developmental, emotional or behavioral.
  • No cost to families.

  • Hosts specialty care clinics for children and youth locally to include Pediatric Neurology and Pediatric Orthopedic (coming fall 2015).
  • Sliding fee scale based on family income.

  • Nurse home visitation program. 
  • Serves families with children birth to 6 years.
  • Builds on families' strengths to improve health, well-being and development of children.
  • No cost to families.
